In the United States, more than 20 million people have diabetes. Both types of diabetes are characterized by ophthalmic conditions related to the duration of diabetes, blood glucose control and hypertension. The major microvascular complication is diabetic retinopathy. Studies by the ophthalmic community established indications and techniques for laser therapy to prevent loss of vision. More recent studies have indicated that intensive glucose control will significantly reduce the risk of progressive diabetic retinopathy. Intensive control of blood pressure will also slow progression of retinopathy as well as reducing the risk for renal failure and cardiovascular complications. The ophthalmologist has a unique opportunity to work with general medicine and endocrine colleagues in reinforcing the value and strategies of glycemic and blood pressure control in patients with diabetes. This module reviews the rationale for intensive management of glycemic and blood pressure control in diabetes mellitus. Discussion of other important management tools is beyond the scope of this module
