Summary
This module reviews the clinical features, pharmacology, and mechanism of Intraoperative Floppy Iris Syndrome (IFIS), as well as options for clinical and surgical management. In eyes with IFIS, cataract surgery is more difficult, and the risk of complications is higher if the surgeon does not anticipate or recognize this syndrome. In addition to a tendency for poor pupil dilation, classic IFIS manifests a triad of intraoperative signs: iris billowing and floppiness, iris prolapse to the main and side incisions, and progressive miosis. IFIS can be caused by systemic alpha1-antagonists, which are commonly prescribed for patients with benign prostatic hyperplasia (BPH)
