This module covers the prevention, management, operative considerations, various surgical approaches, postoperative management and complications of dislocated IOLs. It includes four surgical videos, plus photographs and helpful step-by-step figures for techniques. There has been tremendous progress in cataract surgery since the first intraocular lens implantation. It is now well established that a combination of continuous curvilinear capsulorrhexis, phacoemulsification, and in-the-bag placement of an intraocular lens (IOL) represents a standard of care for modern-day cataract surgery. Careful identification of pre-existing factors will alert the surgeon to take extra care during intraocular lens surgery. If a subluxed or dislocated IOL has been identified, surgery may not always be necessary. If surgery is indicated, careful preoperative and intraoperative assessment are critical in choosing what surgical approach is best suited to each patient. Various options exist, and choosing the safest operation with the fewest potential complications is recommended. IOL explantation and exchange should be reserved for those patients where simple repositioning or refixation techniques would not be sufficient
