Presbyopia is the progressive, age-related loss of the eye's accommodative ability. The precise physiology of accommodation and pathophysiology of presbyopia is still unknown. Alhough spectacle corrections have long dominated the options for presbyopia correction, recent technological innovations have vaulted the management of presbyopia into the surgical arena. No current technology is able to fully restore the accommodative power of the eye, but a variety of surgical treatments for presbyopia restore some level of near and intermediate vision while maintaining good distance vision acuity. This module reviews several key theories of accommodation and presbyopia and overviews surgical methods for presbyopia treatment, including monovision correction, scleral-based surgery, corneal-based surgery, lens-based surgery, preoperative considerations and the management of complications.
