- Optic atrophy (OA) is not a specific etiologic entity; it is the final common pathologic endpoint of any disease that affects the retinal ganglion cell layer or its axons (the retinogeniculate visual pathway). OA refers to the permanent involution of the optic nerve resulting in an altered appearance, or pallor, of the optic nerve head. The degree of optic disc pallor does not reliably predict visual function or etiology, so the clinical value of observing pallor comes only when it is considered in the context of clinical history and sensory visual function. This module describes the approach the ophthalmologist might take in making a differential diagnosis of OA
