- The commercial availability of newer drugs that are well tolerated and efficacious for the majority of common bacterial corneal and external eye infections has revolutionized the treatment of these diseases, increasingly shifting the responsibility for treatment decisions to the primary clinician. The small margin for error in ocular infections demands adherence to general principles of antibiotic use and requires a working knowledge of likely pathogens, antibiotic spectrum of activity, drug toxicity, and expected clinical course. Significant diagnostic delay, variable yield, and, often, inexact identification limits the utility of current microbiologic methods for external and corneal infections. The ongoing development of molecular diagnostic techniques promises rapid, precise detection of the genetic material of potentially pathogenic organisms, rendering initial antibiotic choice more effective and further refining our understanding of antibiotic sensitivity and resistance
